Daycare Admission Requirements

 

A child may not be permitted to enroll into the program if any of the following forms and/or fees is not on file/paid

Required Forms

  • Application Form
  • Certificate of Immunization (Blue Card
  • Emergency Card
  • Medical Form
  • Parent-Center Agreement
  • Notarization Form

Application/Supply Fee

  • $15.00 Non-refundable application fee
  • $25.00 annual supply Fee, Due no later than September 1st.
